Stem Cell Scaffold Enhances Vision in First Human Trial for ARK
A first-in-human trial at Moorfields Eye Hospital and University College London showed that a dual stem cell scaffold improved vision and eye surface scores in nine patients with aniridia-related keratopathy, a previously untreatable condition. The study published in JAMA Ophthalmology indicates significant improvements in ocular surface scores and vision metrics after treatment.

In a groundbreaking first-in-human clinical trial conducted at Moorfields Eye Hospital and University College London, researchers demonstrated that a dual stem cell scaffold significantly improved vision and ocular surface scores in patients suffering from aniridia-related keratopathy (ARK). This progressive eye disorder, characterized by the loss of limbal stem cell function, leads to abnormal tissue growth across the cornea, resulting in cloudiness and potential vision loss. The trial involved nine participants who received this innovative treatment, marking a significant advancement in addressing a previously untreatable condition.
Aniridia-related keratopathy is a genetic condition caused by mutations, particularly in the PAX6 gene, affecting approximately one in 40,000 to one in 100,000 individuals. In ARK, a shortage of limbal stem cells allows conjunctival tissue to invade the cornea, worsening vision over time. Researchers at Moorfields and UCL developed a collagen scaffold designed to deliver limbal epithelial stem cells and stromal stem cells directly to the affected areas of the eye, offering a potential solution to restore function and clarity.
The trial's results were promising. Initially, the average ocular surface score (OSS) for treated eyes was 9.4, indicating moderate impairment, while untreated eyes had an OSS of 8.4. After three months of treatment, the OSS for treated eyes improved to 5.9 and further to 6.7 after twelve months. In contrast, untreated eyes showed minimal change, reaching an OSS of 8.0. Additionally, vision tests demonstrated significant improvements, with participants experiencing an average increase of 24 letters on the ETDRS vision test, equating to nearly five lines on a standard vision chart.
Key stakeholders in this trial included Professor Sajjad Ahmad, a consultant ophthalmic surgeon at Moorfields Eye Hospital and professor of corneal regeneration at UCL, who emphasized the significance of this dual stem cell approach. The success of the trial has prompted plans for larger clinical trials, aiming to further validate the treatment and potentially secure its approval as a standard NHS treatment. This research received funding from the Medical Research Council, Moorfields Eye Charity, and the NIHR Biomedical Research.
